NEW TEAM APPLICATION FORM

NOTE:

1) The Annual Registration fee includes the state mandated individual player registration fee for up to 22 players. Any additional players that are added over the 22 to a teams roster over the course of the season will be billed to the team and is to be paid during the mid season break and again at the end of the season.

2) The Game Bond is refunded to the team once they terminate their involvement with the league and provided the team is in good financial standing.

3) The Two Year Bond is refunded to the team after two consecutive years with the league. The league will subtract any outstanding balance that the team holds from that refunded amount. If a team continues it's involvement with the league at the request of the team the amount can alternatively be held by the league and applied to any future fees that may occur.
